About Baobab Care
Our services provide for individuals who have Learning Disabilities, Autism Spectrum Disorders, Mental Health Conditions, Substance Misuse and Enduring Conditions.
We deliver Care and Support Services personalized to meet individual needs and requirements.
Access to our services are based upon the criteria of the Health and Social Care Act 2008 Regulated Activities and the Care Act 2014 therefore allowing service users to access a range of daily living activities which ensure their well-being and are necessary to remain independent and productive citizens.
Whilst we promote independent living our service is based on the need for assisting and prompting with personal care and support activities, which can include, personal hygiene, dressing, meal preparation, medication. Including any other day to day activities, such as, assisting to undertake shopping, domestic or laundry duties, assisting with benefits, social activities and escort duties to colleges, work placements and other community facilities.

The support provided is person-centred and specifically tailored to meet every unique need, underpinned by our philosophy that every individual has the right to live the life they choose for themselves.

How how process works
Step # 1
To move to a supported living service you will need to have funding in place. Contact your local council or GP who will put you in touch with a social worker to help get your support agreed.
Step # 2
Once your local council has agreed your support, you can then get in touch with Baobab care. One of our Referral Managers will then get in touch with you.
Step # 3
We’ll arrange a visit in person to get to know you a little better. We’ll talk about your current situation, support need and also your short, medium, and long term ambitions.
Step # 4
We’ll arrange visits to our available apartments or shared houses in your chosen location so you can have a look around and ask our team any questions you might have.
Step # 5
We’ll create you a support and funding proposal and send it to your local council for approval – keeping you up to date throughout the process.
Step # 6
Once funding is approved we can set a move in date, and will be on hand to support you through the whole transition process to make the move as smooth as possible.
